For all of our customers, who choose to use a credit card as their form of payment for the purchase of products from  Key Marketing, we need the following information for our records.  This is for the protection of both your company and our in case of credit theft or fraudulent credit cards that are being produced by credit card rings nationwide.

This form will only need to be completed, signed, and faxed back to our office for the processing of your initial credit card purchase from  Key Marketing.  For subsequent purchases, this information and your authorized signature will be on file with us.
